Enrollment to VA Health Facility is required. VA Form 10-10 EZ must be completed and submitted to the VA Medical Center, 4100 W. Third, Dayton, Ohio 45428. After completion of form you must then contact the Lima Community Based Outpatient Clinic for an appointment (wait ten (10) working days prior to contacting the CBOC) Contact the CBOC at 419-227-9672.
